Probably the most facepalm-inducing incident involved the lyrics of "Komm Suesse Todt" in "End of Evangelion". The first time I heard it (when I saw the movie for the first time, of course) I misheard the line "letting me down" (in the refrain) as "let it rain down". Personally, much as I love the song as it is, I like my version better: I guess I was hoping all those souls being absorbed into the Black Moon would "rain down" back to earth and take on their human forms again, so you can imagine how embarrassed and irritated I was when I found the actual lyrics.
